The Bluebell Unit at Mapleton Residential Home is a 10 bedded unit focused on supporting people with recovery and rehabilitation following a hospital admission. Working in collaboration with the person, we develop individualised rehabilitation goals to support people to recover to their own homes.
We are looking for a Social Care Assessor to join the established multidisciplinary team in completing strengths-based Care Act Assessments and personalised care plans to support individuals when their long-term care needs have been identified. You will also be responsible for the co-ordination of care and ongoing social care support including providing information, advice and guidance where required. You will be required to complete mental capacity assessments and support in gathering information relating to safeguarding concerns when appropriate.
This is a rewarding role, pivotal in supporting people to return to their own homes by building and maintaining independence and connecting people to their local communities.
About you
This role would suit someone looking for a fast paced and varied role. You will be experienced in working with people in a health or social care environment. You will have good verbal and written communication skills, with the ability to complete high quality assessments. The nature of this work requires you to be highly organised with strong recording skills; You will need to have the ability to prioritise assessments and manage competing demands simultaneously.
You will have a flexible approach and be a team player who is able to forge excellent working relationships with multi-disciplinary teams and partner agencies. You will work to empower individuals to take control through personalised, strengths-based models of assessment and intervention; putting people at the centre of everything we do.
About the Team
You will be a member of the Southern Devon Hospital Discharge Team, where you will be supported with regular supervision, peer supervision groups and learning opportunities to support you with progressing your social care career. The Team holds a varied caseload, supporting people who are currently inpatients as well as those who have recently been discharged; undertaking assessments to identify ongoing care and support needs. We are an Integrated Service working closely with colleagues in the NHS, Torbay Adult Social Care, Short Term Services Teams, the Voluntary Sector as well as our community-based colleagues.
As part of day-to-day duties, you will work closely alongside the Registered Manager and care staff at Mapleton as well as colleagues in short term services, intermediate care teams and the voluntary sector.
Being part of this team will require you to demonstrate flexibility, adaptability, and a commitment to continuous improvement in service delivery.
