We are looking for a positive and motivated Social  Care Assessor to join our integrated Adult Social Care team covering the West area of Devon.

Based in our Tavistock office with flexibility to work from home as part of our hybrid working policy, you will undertake a key Social Care Assessor role within our Adult Social Care Team. The post is a full-time, permanent position requiring office-based working at least three days per week.

You will be working with Adults aged from 18 to people who are Over 65 and who present with a range of physical and / or learning disabilities as well enduring health conditions, frailty and dementia. You will be working as part of the wider Community Health and Social Care Team, consisting of Experienced Social Workers, Social Care Assessors, Occupational Therapists and Community Nurses.

If you like a fast paced and varied role this job will suit you. You will be writing  strengths based social care assessments, coordinating commissioned packages of care, arranging placements in 24 hr care and providing ongoing social care support in complex case work and reviews. You will be required to raise safeguarding adult concerns where necessary. There is a requirement to support CHC applications, an expectation to undertake Mental Capacity Assessments and become involved with Best Interest decisions and support Court of Protection applications.

The nature of this work requires you to be highly organised with strong IT and recording skills and in turn you will receive excellent support in a busy and friendly team.

You will become familiar with key policies and legislation and help to ensure the Local Authority is meeting its duties and responsibilities under The Care Act and other legal frameworks.

You will attend training on safeguarding and other essential areas of practice.

You will have had experience in working with people in sometimes difficult situations, their carers, other professionals, and with managing conflict. You will build on communication skills and become effective communicating both verbally and through written formats.

You will have developed skills in managing and prioritising your workload autonomously and work well as part of a caring and hardworking team. You will have good time management skills and be able to work under pressure.

You need to be IT literate and willing to develop your skills.
