The role

We require a forward thinking, innovative lead nurse to provide clinical leadership and management to our Continuing Healthcare Commissioning team in South, Teign and West Devon.

This is a dynamic post that requires the right person with excellent clinical leadership and management skills to lead the Continuing Healthcare Commissioning Team to help achieve the best outcomes for people with learning disabilities, enabling those with the most complex of needs to remain within their own homes and lead fulfilled lives within their local communities.

£38,890 – £44,503 per annum pro rata (Band 7)

About you
You must have excellent communication skills and ability to foster close working relationships with other partnership agencies and service providers, alongside other community learning disability teams in your locality to provide a seamless and comprehensive service to people with a learning disability and their carers.
