We are looking for an enthusiastic and motivated Community Occupational Therapist to join our Community Health and Social Care Team in Tiverton on a 12-month fixed-term contract.
This is an excellent opportunity for a qualified Occupational Therapist who is passionate about supporting adults to remain as independent as possible within their own homes and communities. Working as part of a multidisciplinary team, you will undertake strengths-based assessments, identify practical solutions to promote independence, and support individuals to achieve outcomes that matter to them. You will work with adults with a wide range of physical disabilities, health conditions and support needs, providing specialist Occupational Therapy assessment, advice, equipment recommendations, moving and handling solutions, and adaptation recommendations
About the Role
You will:
- Complete person-centred Occupational Therapy assessments.
- Promote independence through a strengths-based approach.
- Assess for and recommend equipment, minor and major adaptations.
- Undertake moving and handling assessments.
- Work collaboratively with colleagues across Health, Social Care, Housing and voluntary sector organisations.
- Contribute to safeguarding responsibilities and Care Act assessments where appropriate.
- Manage a varied caseload with support and supervision from experienced Occupational Therapy colleagues.
About You
We are looking for someone who:
- Holds a Degree or Diploma in Occupational Therapy.
- Is registered with the Health and Care Professions Council (HCPC).
- Has excellent assessment, communication and problem-solving skills.
- Is committed to person-centred and strengths-based practice.
- Works effectively as part of a multidisciplinary team.
- Is able to prioritise workload and manage competing demands.
