About the Team

The Moor to Sea Community Health and Social Care Team support clients to return to the community following admission to hospital. We assess care pathways to support a return home or or long term provision to meet people’s needs, working closely with Multidisciplinary Teams across community and acute hospitals.

The Team have a varied caseload supporting people who have recently been discharged; undertaking assessments to identify ongoing care and support needs.

We are an Integrated Service working closely with colleagues in the NHS, Torbay Adult Social Care, Short Term Services Teams, the Voluntary Sector as well as our community based colleagues.

We support a wide range of service user groups including but not limited to;
• Older People
• People with Primary Mental Health Needs
• People with Learning Disabilities
• People receiving Rehabilitation
• People affected by Substance Misuse
• People receiving End of Life Care
• Younger Adults
• Carers

About the role

This is a rewarding role, supporting people to recover and rehabilitate following a period of illness or injury. The role requires professional judgement, empathy, and a strong commitment to equality and connecting people to their communities to live as independently as possible.

You will work within the discharge to assess model; completing strengths based referrals to support people in their discharge from hospital settings. Working alongside multidisciplinary team colleagues, you will promote independence and, using an asset based approach, seek to reduce and delay the need for formal care and support.

This role includes the completion of strengths based Care Act Assessments and Personalised Care Plans. You will also be responsible for the co-ordination of care and ongoing social care support. You will be required to complete mental capacity assessments and support in gathering information relating to safeguarding concerns when appropriate.

In line with Devon County Council Guidance, this role offers a mix of home based and office based working.

A social worker looking to develop their skills and knowledge around hospital discharge process and health multi-disciplinary involvement.

You will bring to the role;
– Experience in working with people in a health or social care environment
– Excellent verbal and written communication skills, with the ability to complete high quality assessments.
– The ability to work both independently and within a team.
– The nature of this work requires you to be highly organised with strong recording skills;
– The ability to prioritise referrals and manage competing demands simultaneously.

Social Worker Career Pathway  

We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway.     

This is a fantastic opportunity to provide you with an achievable, clear pathway for your career; to enable you as a practitioner to reflect and identify your own practice and development on the PCF pathway.  

If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).  

With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
