We serve adults from the ages of 18 upwards including people with physical disabilities, older people, and people with Learning Disabilities. We are part of an integrated community team and as a Social Care Assessor you will be working closely with our health teams to support people to improve their life outcomes.

What we’re looking for 

We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers and other professionals. You will have experience of working in a health or social care environment and have a sound understanding of human needs. You will also ideally have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team. You should be an effective communicator, both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have effective time management skills and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to effective customer service and to achieving positive outcomes for vulnerable people. In return, you will be part of a friendly, supportive and experienced team with regular supervision, regular team meetings and access to learning and development opportunities.
