Are you passionate about supporting people in our community to live as independently as possible? We have an exciting opportunity for 3 full time Social Care Assessors to join Care Direct Plus South Assessment and Review Team based at Forde House in Newton Abbot. We are a dynamic and progressive team, delivering the adult social care front door for our communities.

The Assessment and Review Team are the first point of contact to discuss any challenges people are facing with living independently providing support via the telephone. As a Social Care Assessor, you’ll play a vital role in supporting people with various disabilities, older people, and carers using a strengths based approach, providing information and advice, signposting, assessing needs, planning support, and working collaboratively with individuals, carers, professionals, and partner agencies.

As a Social Care Assessor within this team you will be required to work Saturday’s on a rota basis. This is a hybrid role and you will be required to work in the office which is based in Newton Abbot for 50% of your working week.

Key Responsibilities:

Carry out holistic assessments via a strengths based approach and develop person-centred support plans via the telephone.
Provide advice, guidance, and information to help people maintain independence. Work closely with other services to ensure seamless support.
Make appropriate referrals and review care arrangements to ensure positive outcomes.
To be able to use the work systems in place to record your conversations.

What We’re Looking For:

Experience in a health or social care setting in a fast paced environment.
Confidence in working with individuals in challenging situations.
Strong communication skills – both written and verbal.
Strong person centred and strength based practice skills.
Ability to manage your own workload and contribute effectively to a team.
IT literacy and a willingness to develop digital skills.
A commitment to equality, diversity, and excellent customer service.
