Transitions Social Worker – Mental Health – Ref 7163

We are seeking a compassionate, dedicated, and experienced Mental Health Adult Transitions Social Worker to join our services. This role is essential in supporting individuals with mental health needs as they transition from children’s to adult services. You will work directly with young people, families, and a multidisciplinary team to ensure a smooth, person-centred transition experience that promotes independence, dignity, and holistic well-being.
The role is social worker assigned to DPT and will be based/ working alongside the TASC team in Exeter. The role will be essential in liaising with CAMHS, children’s services and supporting with promoting a smooth and planned transition for young people presenting with mental health as their primary need or with dual diagnosis. The role will receive practice support from DPT Social Care as well TASC.
TASC has developed strong links with Children’s Services and Education and work with young people from the age of 14 up until 25 across the Devon County Council footprint. The team comprises of two Team Managers, Social Workers, Social Care Assessors, Occupational Therapists and Business/Information Support Officers.
The role will work to the Care Act principles to engage with young people with needs relating to their mental health, ensuring that independent living skills are promoted and maximised to facilitate young people’s ability to engage with their community, gain employment, and live and enjoy their adult life as independently as possible.
This is an exciting time for the transitions services in Devon with the TASC project’s overall aim is to improve our joint services in supporting young people in Devon as they plan and make decisions about what their adult life will look like. Over the first year of the project we have shown that effective collaboration with Children’s Services, Education and voluntary agencies is improving the outcomes for young people, and those that care for them, as they transition into adulthood.
We are looking to recruit a full time Social Worker to join our assigned social care workforce and work with the TASC team. It really is a very crucial time in the project development and the successful applicant for this post will play a significant part as the transition work continues to evolve.
About You:
- An experienced and qualified Social Worker (CQSW, DipSW, BA Hons, MA or equivalent) who is registered to practice with Social Work England.
- You will have completed the Assessed First Year in (post-qualifying) Social Work practice, if appropriate, and can demonstrate a commitment to ongoing professional development as we are committed to supporting you to look at post-qualifying opportunities during your career with us.
- Bringing a comprehensive working knowledge of the issues currently impacting on social care delivery, you will be up-to-date with policy and practice development.
- You will need to hold a current full driving licence and have access to a vehicle due to the requirement to travel significant distances as the role is Devon Wide.

Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway.

If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).
With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
