We are seeking an experienced Social Care Assessor to join our supportive and forward thinking Adult Social Care team.
About the Role
In this full time position, you will play a key role in delivering high quality, person centred social work practice. You will work with adults with a range of complex needs, empowering individuals to live independently and safely while ensuring statutory duties and best practice standards are met.
Key Responsibilities
Undertake strengths based assessments and develop personalised care and support plans.
Work collaboratively with health partners, voluntary services, carers, and families.
Manage a varied and complex caseload with confidence and professionalism.
Apply legislation including the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
Make defensible decisions that promote wellbeing and positive outcomes.
Maintain accurate and timely records in line with organisational and professional standards.

* Skilled in assessment, risk management, and a good understanding of safeguarding adults.
